BY SIGNING ABOVE, THE PERMIT HOLDER ACKNOWLEDGES THAT ALL INFORMATION IN THE APPLICATION IS CORRECT. A LATE FEE OF $50 WILL BE ASSESSED JULY 1st AND AN ADDITIONAL $50 WILL BE ASSESSED AUGUST 1st. AS OF SEPTEMBER 1st, THE MOORING REVERTS TO THE TOWN OF PROVINCETOWN FOR POSSIBLE REASSIGNMENT.
